A Breakout Year for Codoxo and the Next Era of Payment Integrity

By Musheer Ahmed, PhD, Founder & CEO, Codoxo

Over the past year, I’ve engaged in numerous discussions with health plan pharmacy benefit managers (PBM), and one recurring theme has been unmistakable: the current payment integrity model is no longer sustainable.

The affordability of healthcare continues to worsen. Administrative burden waste continues to climb healthcare system. Claims volumes are rising. Provider abrasion is worsening. And too often, the industry is still trying to fix problems after the fact. After a claim is already submitted. After waste has already occurred. After claim payments payment errors have been made. After relationships have already been strained.

At Codoxo, we believe there is a better way forward.

This past year marked a defining moment for our company as we delivered payment integrity solutions and services that break the status quo. New innovation that allows payers  to move further upstream, avoid costs, create workflow efficiencies and partner with providers. The market’s response has been overwhelmingly positive, and we are grateful to our partners for their belief in us. Check out the highlights of our 2025 success below.

A Breakout Year of Growth and Validation

I’m proud to share that Codoxo recently closed an oversubscribed $35 million Series C funding round, led by CVS Health Ventures, with participation from Echo Health Ventures and continued support from our existing strategic and financial investors.

This investment is more than capital. It’s validation that the industry is ready for a fundamentally new approach to cost containment.

With this round, we’re accelerating national adoption of our newly released Point Zero Payment Integrity and expanding the pace of innovation across our generative AI-powered platform.

Point Zero: Redefining Payment Integrity at the Earliest Stage

Traditional payment integrity operates in prepay or postpay environments, catching issues after the system has already absorbed unnecessary cost and friction.

Point Zero, our new approach that launched in 2025, changes the model entirely.

It introduces proactive, AI-driven provider education and intervention before claims are ever created. By addressing payment accuracy at the earliest possible stage, we help payers prevent errors at the source, rather than managing them downstream.

Organizations using Point Zero are seeing meaningful impact, including:

  • $12–$16 PMPY in savings through early intervention
  • 40% drop in overpayments
  • 20-30x greater efficiency
  • 90% improvement in workflow automation
  • Less provider abrasion from delayed payments, denials and appeals

This is what prevention-first payment integrity looks like in practice.

Continued Revenue Growth & Scaled Financial Impact

Over the past year, Codoxo has demonstrated impressive year-over-year revenue growth and significantly expanded its customer base. The company’s Unified Cost Containment Platform now enhances payment integrity for over 80 million members. In 2025, Codoxo proudly achieved 100% customer retention and 125% net revenue retention, signaling strong and growing adoption within its existing client portfolio. Customer implementations are yielding significant results, with cost reductions of up to $66 per member per year across the payment spectrum, all while and minimizing downstream denials, appeals, and friction with providers.

Breakthrough Innovation Powered by Generative AI

Healthcare fraud and payment complexity continue to evolve, and so must the tools we use to address them.

This year, Codoxo introduced several major innovations across our gen AI roadmap, including:

  • The Data Health Integrity (DHI) Agent, enabling payers to share high-fidelity claims data without costly delays
  • A new Policy-to-Code capability, using generative AI to convert complex payer policy language into executable code
  • Deepfake Detection, identifying synthetic medical documents and manipulated imagery with AI-driven precision fraud detection

These advancements reflect our commitment to building artificial intelligence that is not only powerful, but practical: reducing time to go live, increasing accuracy, and staying ahead of emerging fraud threats.

Enterprise-Grade Security and Compliance

Innovation in healthcare demands operating at the highest level of safety and security and in 2025 Codoxo continued its commitment through:

  • NIST 800-53 Revision 5 compliance for the second consecutive year
  • Continued HITRUST r2 certification
  • Progress toward FedRAMP readiness

Security and trust remain foundational as we scale nationally.

Recognition, Partnerships, and a Growing Ecosystem

It has also been incredibly meaningful to see Codoxo’s impact recognized across the industry, including honors such as:

We also expanded strategic partnerships, including collaboration with HealthEdge, bringing advanced payment integrity capabilities directly into core payer platforms.

Investing in Leadership and Community

Momentum is never built by technology alone – it’s built by people.

This year, we welcomed outstanding new leaders, including:

We also continued expanding our role in shaping the future of healthcare AI through:

Looking Ahead

This past year marked a turning point for Codoxo and for the broader shift toward prevention-first healthcare cost containment.

By combining generative AI with proactive provider engagement, we’re proving that payment integrity can deliver measurable savings and stronger payer-provider relationships.

The momentum we’ve built is just the beginning.

Thank you to our customers, partners, investors, and the entire team for believing in what Codoxo can unlock for healthcare.

The work ahead is about continuing to raise the standard for what payment integrity can achieve for containing healthcare’s costs – through AI, collaboration, and lasting impact.
