Early Monitoring Opportunities for 2027 CPT Code Updates

The AMA announced 299 new CPT codes, 74 revisions, and 80 deletions for the CPT 2027 code set, effective January 1, 2027. Key updates focus on maternity care, ventricular assist devices, hernia repair, prostate biopsy, biofeedback, radiology, sleep medicine, and additional AI-related services.

For Special Investigation Units (“SIU”), payment integrity teams, and provider analytics organizations, the introduction of these new codes creates an opportunity to proactively assess emerging fraud, waste, and abuse (“FWA”) vulnerabilities. This article highlights selected CPT 2027 categories where early claim monitoring, documentation review, and provider comparison may help distinguish legitimate adoption from inappropriate coding patterns.

Maternity Care Services

What changed

One of the most transformative updates in CPT 2027 is the replacement of the longstanding global maternity care bundle with a revised coding framework designed to better reflect modern, team-based obstetric care. The new structure recognizes the reality that maternity services are increasingly delivered across multiple providers, care settings, and specialty disciplines rather than through a single physician managing the entire episode of care. This change improves flexibility, enhances attribution of services, and allows healthcare stakeholders to better measure care delivery throughout pregnancy, labor, delivery, and postpartum management.

Why it matters

The revised structure may improve service attribution across pregnancy, delivery, and postpartum care, but it also increases the need to monitor how providers separate, sequence, and bill services that were previously captured under broader global maternity billing concepts.

SIU/FWA review concepts:

Unbundling and duplicate billing risk

  • Monitor claims where former global maternity services appear to be billed as multiple separate services or by multiple providers during the same pregnancy episode.
  • Compare total reimbursement per pregnancy episode before and after implementation to identify abrupt cost escalation or overlapping antepartum and postpartum billing.

Biofeedback Services

What changed

A significant change within the CPT 2027 code set is the creation of a time-based coding structure for biofeedback services. The new framework aligns reimbursement methodology with how biofeedback therapy is typically delivered in practice, providing a more accurate reflection of clinician time and patient engagement. This change appears to be intended to facilitate improved reporting consistency across rehabilitation, behavioral health, and pain management settings.

Why it matters

Because the new framework is time-based, SIU review should focus on whether billed duration, provider capacity, clinical documentation, and concurrent service patterns support the claimed level of service.

SIU/FWA review concepts:

Time and capacity validation

  • Identify providers consistently billing maximum durations or aggregate daily treatment time that exceeds reasonable provider availability.

Documentation and delivery setting review

  • Review place-of-service consistency, measurable therapeutic objectives, and overlapping time-based services billed on the same date.

Sleep Medicine

What changed

Six (6) new CPT codes for unattended sleep studies reflect advancements in remote diagnostic technologies and an increasing shift away from traditional sleep laboratories. These updates recognize the growing role of home-based testing solutions and help establish clearer reporting pathways for technology-enabled sleep diagnostics.

Why it matters

Expanded home-based testing pathways may improve access and convenience, but they also require monitoring for rapid volume growth, repeat testing, equipment capacity concerns, and referral patterns that may indicate overutilization or inappropriate diagnostic activity.

SIU/FWA review concepts:

Utilization and testing pattern review

  • Monitor abrupt home sleep study volume spikes, multiple unattended studies in short periods, and testing frequency that exceeds clinical expectations.

Referral, equipment, and treatment conversion analysis

  • Evaluate whether sleep study claims align with device inventory capacity, expected referral patterns, and clinically reasonable test-to-treatment conversion rates.

Artificial Intelligence (AI) Services

What changed

The CPT 2027 code set adds ten (10) new AI-related codes, increasing the total number of AI-related CPT codes to forty-three (43). The AMA also updated its Taxonomy for Artificial Intelligence in Medical Services and Procedures to help stakeholders apply consistent terminology as AI technologies continue to emerge. These changes underscore the growing role of AI in supporting healthcare professionals across diagnostic, clinical, and administrative workflows while ensuring coding standards remain aligned with technological innovation.

Why it matters

As AI-enabled services expand, payors will need to validate whether the billed service reflects actual use of qualifying technology, whether the AI output informed clinical decision-making, and whether reimbursement duplicates services already reported through traditional codes.

SIU/FWA review concepts:

AI service validation and documentation support

  • Confirm whether claims include evidence of AI platform utilization and documentation showing how AI outputs informed clinical decisions.

Vendor concentration and cost escalation

  • Evaluate relationships between AI vendors and high-utilizing providers while measuring incremental spending attributable to AI-enabled services.

As the CPT code changes become effective, payors and SIUs should view these updates not only as coding changes, but as early indicators of where new billing behaviors may emerge. By tracking adoption patterns, comparing provider behavior to peer norms, and validating documentation against billed services, organizations can strengthen oversight while supporting appropriate use of new and evolving service models.

How Fraud Scope can assist:

Rather than treating the CPT changes solely as a coding update, healthcare organizations can use Fraud Scope to establish baseline utilization patterns, monitor adoption trends, and identify providers whose billing behavior materially differs from that of their peers.  For example, a provider that rapidly shifts a significant percentage of historically billed procedures to newly introduced CPT codes may warrant further review to determine whether the new coding is supported by documented services or influenced by reimbursement incentives.

The New Utilization of Codes view within the Highlighted Provider Activity widget provides insight into how providers adopt new codes relative to their peer groups.  Users can identify rapid growth in CPT code billing, particularly when a code has not previously appeared in a provider’s utilization profile.  Fraud Scope’s query toolkits also allow users to create targeted reports that identify utilization spikes and trends and provide the supporting line-level claims data.  AI Query, Fraud Scope’s generative AI query tool, can help partners quickly identify adoption of new CPT codes and detect shifts in utilization from historically billed codes to different codes by the same provider.
